MEDICATION THERAPY MANAGEMENT (MTM) SERVICES ARE THE EXPERIENCED, MEASURABLE EVENTS WHEN PHARMACEUTICAL CARE IS PRACTICED.

Medication management services are the professional activities needed to meet the standard of care which ensures each patient’s medications (whether they are prescription, nonprescription, alternative, traditional, vitamins, or nutritional supplements) are individually assessed to determine:

1. Each medication is appropriate for the medical condition being treated.

2. Each medication is being effective and achieving the goals established.

3. Each medication is safe for the patient in the presence of the co-morbidities and other medications the patient may be taking

4. The patient is able and willing to take each medication as intended.

This assessment is completed in a systematic and comprehensive manner.

In addition to the comprehensive assessment of the patient’s drug-related needs, medication management services include an individualized care plan that utilizes the patient’s medication experience and preferences to determine desired goals of therapy with the patient, as well as appropriate follow-up to evaluate actual patient outcomes that result from the care plan. This all occurs because the patient understands, agrees with, and actively participates in the treatment regimen, thus optimizing each patient’s medication experience and clinical outcomes.  Medication management services must be delivered and documented in a manner that adds unique value to the care of the patient and integrates easily with the medical team caring for the patient.
